If you do not agree to these terms, please do not use our services. These terms constitute a legally binding agreement between you and Sigla Primary Care & Wellness. 02 ## Eligibility Sigla Primary Care & Wellness services are available exclusively to individuals who are at least 18 years of age and are residents of the state of Texas. By using our services, you represent and warrant that you meet these eligibility requirements. Our clinicians reserve the right to determine medical eligibility for any treatment program. 03 ## Telehealth Services Sigla Primary Care & Wellness provides telehealth consultations through licensed medical professionals. While telehealth is an effective method of healthcare delivery, it is not a substitute for emergency medical care. If you are experiencing a medical emergency, please call 911 or visit your nearest emergency room immediately. 04 ## Prescription Medications All prescriptions are issued at the sole discretion of our licensed clinicians based on your medical evaluation. Sigla Primary Care & Wellness does not guarantee that any specific medication will be prescribed. Medications are dispensed through licensed U.S. compounding pharmacies and are intended solely for the patient to whom they are prescribed. Sharing, selling, or distributing prescribed medications is strictly prohibited. 05 ## Payment and Pricing All prices are listed in U.S. dollars and are subject to change. Payment is required at the time of service. We accept major credit cards and debit cards. Sigla Primary Care & Wellness does not accept health insurance. All fees are clearly disclosed prior to your consultation, and there are no hidden fees, membership charges, or long-term contracts. 06 ## Patient Responsibilities - Provide accurate and complete medical history and personal information - Follow your prescribed treatment plan as directed by your clinician - Report any adverse reactions or side effects promptly - Attend scheduled follow-up consultations - Notify Sigla Primary Care & Wellness of any changes to your health status or medications 07 ## Intellectual Property All content on the Sigla Primary Care & Wellness website, including text, graphics, logos, images, and software, is the property of Sigla Primary Care & Wellness and is protected by applicable copyright and trademark laws. You may not reproduce, distribute, or create derivative works from any content without prior written consent. 08 ## Limitation of Liability Sigla Primary Care & Wellness and its clinicians, employees, and agents shall not be liable for any indirect, incidental, special, or consequential damages arising from or related to your use of our services. Our total liability shall not exceed the amount paid by you for the specific service giving rise to the claim. 09 ## Modifications to Terms Sigla Primary Care & Wellness reserves the right to modify these Terms and Conditions at any time.
